VSync is matching the games graphical refresh rate to that of your monitors refresh rate. If a game is running at 120 FPS and your monitor has a refresh rate of 75, your game will slow down the amount of frames it produces to 75, making the image clean and without tears, or two frames put together. Look at the urinals and how they are unaligned that is tearing.
VSync can greatly decrease performance, if your FPS is lower than your refresh rate, by up to 50% Here's my specs for GTAIV
